Change of Name or Address

Occasionally, Academic Services (AS) will need to communicate with you by letter, e-mail or telephone. It is crucial to inform both the University of Washington and the School of Nursing when you have a change of name or address. Changes sent to the University are not automatically forwarded to the School of Nursing.

To update University and School of Nursing records of a change in your address:

1. Update your address and phone number at the School of Nursing by e-mailing Academic Services (sonas at uw dot edu).
2. Update your address and phone number at the UW by accessing Change Address on MyUW at https://sdb.admin.washington.edu/students/uwnetid/address.asp
