Sometimes you will need to provide evidence that you have completed various trainings
to clinical sites, etc. You should already have copies of these in your personal
records outside of campus.
However, if you do not, complete the online form below to request copies of the documents
you need. For security purposes, we will only send this information to your UW e-mail address.
You will receive them as PDF documents, and should keep these for future reference.
We will respond to you within three (3) to five (5) business days. Due to the amount of time
it takes to complete these requests, expedited service is not available.
- This request for is for nursing students at UW Seattle only.
- We do NOT keep copies of your immunization records in your student file due to HIPAA laws
and regulations. You must obtain these from
Hall
Health on UW Seattle's upper campus.
