Each of these compliance pieces must be met by every student that enters the UW School of Nursing.
Several requirements have expiration dates, including immunizations, CPR licensure and RN licensure
(if applicable). It is your responsibility to maintain these records and renew or update
them when necessary and to keep copies of these in your personal records outside
of campus.
To update this information in your student file, bring your updated records to
Academic Services as soon as possible. Immunizations
should be updated through Hall
Health.
A hold will be placed on your student account if a requirement's expiration date has lapsed. Contact
Christine Noyes (cnoyes at uw dot edu) for questions regarding
account holds.
If you do not have copies of these documents, you may submit a request
to receive copies from your student file.

to attend an in person training. If you are a MN, DNP, or Certificate student, Blood
Borne Pathogens you are required to acknowledge that you have been trained and are
accountable for this knowledge. |
