Academic Services Memorandum No. 32
To be eligible for On-Leave
Status, a graduate nursing student must comply with the
policies and procedures outlined in University of Washington
Graduate School Memorandum No. 9 (Revised March 2006),
On-Leave Status: Maintaining Graduate Status.
Steps for Petitioning for
On-Leave Status:
- Obtain a petition card from
School of Nursing Academic Services, Room T310 of the Health
Sciences Building.
- Review policies and procedures
outlined in University of Washington Graduate School Memorandum
No. 9 (Revised March 2006), On-Leave Status: Maintaining
Graduate Status, http://www.grad.washington.edu/Acad/gsmemos/gsmemo09.htm.
- State your reason for taking leave in the 'Reason for On-Leave Status Request' section on the back side of the Petition for On-Leave Status.
- Obtain and state in writing on the petition card, in the 'Reason for On-Leave Status Request' section: "I have received approval from my faculty advisor, Dr. <faculty advisor's name>, to apply to take leave as noted." If possible, obtain your faculty advisor's initials next to this statement.
- Notify and state in writing on the petition card, in the 'Reason for On-Leave Status Request' section: "I am receiving funding from <funding source> and have notified the manager/PI of this funding, <manager/PI's name>, of my application to take leave as noted."
- Obtain required signature.
For the School of Nursing, the petition card must be signed
by the Associate Dean for Academic Services in Room T310
(not the student's faculty advisor).
- Submit signed petition card
to an Academic Services staff member for photocopying for
the student file.
- Submit completed petition
card to the Registration Office (225 Schmitz Hall) no later
than the fifth day of the quarter, with a nonrefundable
fee.
- Upon return to the University
from On-Leave status, re-subscription to the appropriate
student
email list may be required.
