PhD Program Requirements for Final Quarter

The following steps must be taken the quarter you complete your degree:

  1. Register for at least 2 credits.

  2. Be sure you and your Chairperson have requested the appointment of a reading committee by asking the Office of Academic Services to write a letter to the Dean of the Graduate School. You can send an email request for such a letter to keridwen@u.washington.edu.

  3. File the request for final examination form with Graduate School three weeks before the exam is scheduled. This form is available at the Graduate School Forms Web page, and designates that you are ready for the final examination. You will need the date, time, room, and signatures from your committee.

    Be sure all members of your committee have at least two weeks to read your final draft before the examination.

    Your Chair will receive the warrant several days before the defense.

  4. When you have completed your dissertation, submit to Academic Services:
    1. One BOUND, SIGNED copy of your dissertation, or a receipt from the CMU Copy Center indicating that your dissertation has been sent for binding. See format requirements below.
    2. Two copies of your abstract - one in hard copy and one electronically
    3. Signed Doctoral Degree Completion form
    4. Health Sciences photo identification badge and Access card
    5. All keys for offices/carrel rooms in the Health Sciences building
    6. Any equipment you have borrowed during your program of study

  5. Ensure that you have met all Graduate School final requirements, including returning two (2) copies of your dissertation to the Graduate School by 5:00 pm on last day of quarter: A 15-minute appointment with the Graduate School Thesis/Dissertation Adviser is required for final submission. Graduation Ceremony Information

If you are graduating Summer, Autumn, or Winter Quarter and wish to participate in the School of Nursing Convocation the June following your graduation: Convocation information will be sent to the doctoral student email list and posted on the SoN Convocation Website in April. Make a note to check this site and follow the posted instructions in order to participate in the ceremony.

Please note that if you are graduating in Spring Quarter, and wish to have your name included in the program for the University-wide Commencement Ceremony in Husky Stadium, you MUST submit your signed Request for Final Examination to the Graduate School within the first two weeks of April. Check the UW Commencement Website for the specific date, as it is different every year. The names of students who graduated earlier in the Academic Year will be automatically submitted by the Graduate School.

If you are graduating Spring Quarter, you are advised to arrange for family members to arrive at least two days prior to Convocation. Please note that due to space constraints, graduates are asked to bring only four or five family members to the School of Nursing ceremony.

Binding Format Requirements for School of Nursing Dissertations:

Bindery Information
Color: 588 blue;
Color Stamping: Gold;
Standard cover & spine stamping.

Copies should be sent to Erica Winberry at campus delivery box number 357260.

Please note that a bound copy of your dissertation that must be submitted to Academic Services in addition to the two copies required by the Graduate School. Academic Services recommends that you have your dissertation bound at the Communications Building Copy Center, CMU B-042.
