Pre-Orientation Requirements
Listed below are a number of items that you will need to complete before you come to campus in September. You may wish to download the MN pre-orientation checklist to help you keep track of where you are in meeting these requirements.

  1. Create your UW NetID
    You will need your UW student number. Go to https://uwnetid.washington.edu/newid/ and follow the instructions. Once you have your NetID, send an email to Erica Winberry at keridwen at uw dot edu.
  2. Make your immunization appointment
    All incoming Health Sciences students must be screened by the Immunization Clinic at Hall Health by the start of Fall Quarter. You were sent an email about setting up your immunization appointment at the beginning of July. If you have not yet set up your immunization appointment, send an email to Erica at keridwen at uw dot edu, and she will set up an appointment for you on September 28th or 30th.
  3. Complete these required forms

    All of these forms must be brought in to Academic Services on September 21st or 22nd, depending on when you have your Photo ID walk-in session.
  4. Complete HIPAA training online
    All incoming Health Sciences students are required to complete HIPAA training through UWMC, even if you've previously completed HIPAA at another facility.

    You can complete the training any time before orientation. It will take approximately 90 minutes to finish. Follow these instructions to begin the HIPAA training. You are required to submit proof of training completion (a printed completion certificate) during orientation.

    You will need your first name, last name, and student number to log in.
  5. Complete OSHA Hazards Management training online
    All incoming Health Sciences students are required to complete hazardous materials management training. This is an online training program that takes approximately 30 minutes to complete.

    The URL for the training site is http://www.ehs.washington.edu/psotrain/hazwaste/index.shtm.
  6. Subscribe to the nursgrad listserv
    All students enrolled in the MN program are asked to subscribe to the nursgrad electronic mailing list through Mailman, the Web-based e-mail list manager used at the University of Washington. You must use your UW e-mail address/UW NetID and full name to subscribe. If you do not, your subscription and messages will be rejected. Failure to subscribe will cause you to miss crucial information about deadlines, registration, new courses, graduation, etc.

    Go to http://mailman2.u.washington.edu/mailman/listinfo/nursgrad and follow the subscription instructions.
  7. Get your UW ID Card (Husky Card)
    The Husky Card is the official identification card for members of the UW community. Husky Cards are made at the Odegaard Undergraduate library. No appointment is required. Visit the Husky Card website for more information.
  8. Update your personal laptop computer if you are bringing it to campus

Photo IDs & Required Paperwork
During the week before Orientation, you should plan to come to campus and have your picture taken for your Health Sciences photo ID badge. Bring all of the required pre-orientation paperwork with you, and turn it in either before or after your photo ID appointment. The day you should come to campus for your photo ID is determined by your last name.

Bring a completed Photo ID form when you get your Health Sciences student ID.

Bring your payment (personal check or major credit cards ONLY) to the Locke Computer Center in T-271. The student price for an ID on 9/21 and 9/22 is $25.43. The ID cost increases on 9/25. If you wish to purchase an after-hours building access card, the cost for both is $33.52.

After paying, you will go across the hall to T265A, the UWCreative Photography Studio, to have the photo taken. The photo lab will be open on the following dates and times:

Monday, September 21st
10:30am - noon Students whose last names begin with A-L only.
2:00 - 4:00pm
Tuesday, September 22nd
10:30am - noon Students whose last names begin with M-Z only.
2:00 - 4:00pm
Friday, September 25th
By appointment only; contact Academic Services to set up an appointment on this date

You will receive your photo ID and access card (if applicable) the 2nd or 3rd week of the quarter.

Academic Services is located in HSB room T-310. You will need to turn in the following paperwork:

  • Consent to Release Records form
  • UW Medicine Privacy Agreement
  • HIPAA completion confirmation page
  • OSHA Hazards Management training completion confirmation page
  • Blood Borne Pathogen Acknowledgement form
