BSN Student Orientation Information

Read through your orientation materials thoroughly. Make sure that you have completed all of the following before you come to campus in June (summer-start) or September (autumn-start) for your immunization appointment, document check-in and orientation day.

Download and Complete
Return the following forms to Academic Services, and keep a copy of these for your own records:

• Clinical assignments acknowledgement form
• Consent to Release Records
• UW Medicine HIPAA training confirmation (log-in info is given in your orientation e-mail)
• OSHA Hazards Management Training confirmation
• UW Medicine Privacy Agreement

• Attend the immunization clinic on your scheduled date in Hall Health on the Seattle campus (instructions on how to schedule your appointment were sent by email).
• HIV/AIDS course brochure (you may also register online through UWCNE)
• Bring a completed Photo ID form when you get your Health Sciences student ID.
  
  Bring your payment (personal check or major credit cards ONLY) to the Locke Computer Center in T-271. The student price for an ID on 9/21 and 9/22 is $25.43. The ID cost increases on 9/25. If you wish to purchase an after-hours building access card, the cost for both is $33.52.
  
  After paying, you will go across the hall to T265A, the UWCreative Photography Studio, to have the photo taken. The photo lab will be open on the following dates and times:
  
  

  Monday, September 21st
  10:30am - noon	Students whose last names begin with A-L only.
  2:00 - 4:00pm
  Tuesday, September 22nd
  10:30am - noon	Students whose last names begin with M-Z only.
  2:00 - 4:00pm
  Friday, September 25th
  By appointment only; contact Academic Services to set up an appointment on this date

  
  You will receive your photo ID and access card (if applicable) the 2nd or 3rd week of the quarter.
• Establish your UW NetID
• Subscribe to the bsn1yr e-mail listserv
• Purchase the required uniforms in person. Orders will not be taken over the phone.
• Make sure you have a current CPR card, good through June 2010
• Update your personal laptop computer if you are bringing it to campus
