Academic Services Memorandum No. 2
STUDENT EVALUATIONS
Revised September, 2000
1. Student evaluations are submitted by advisors and instructors to Academic Services at the end of each quarter.
2. Evaluation records include the following:
- Course name and number.
- Name of student.
- Name of clinical agency, if applicable.
- Dates, including year and quarter.
- For advisors noting graduate student progress: comment on progress
toward thesis, project or dissertation.
- Any pertinent information about the student's demonstration of meeting
course and/or program objectives.
- Instructor/advisor name and signature.
- Student signature indicating that s/he has read the evaluation.
3. Instructor Responsibility: Discuss quarterly student evaluations with
each student prior to the end of finals week. Review and discuss your
evaluation of the student with him/her during the final evaluation
meeting. After review, sign and have the student sign the evaluation.
4. All student records are to be kept in the student file in the Academic
Services Office in Room T310. Except under unusual circumstances, student
files and/or contents are not to be removed from that office.
