• Trainee Information Form

You will bring these to your site orientation (see below).

2. Site requirements

1. Complete a brief orientation before the start of your clinical rotation at any Pacific Medical Center clinic. Sonya Fukeda will contact you regarding this soon after you receive your official placement confirmation.
2. Get your immunization record and provide to the site on your orientation day. It should show:
   • At least 2 Hepatitis B vaccines
   • TB skin testing results
