1. Site requirements

1. Get a copy of your most recent immunization records from Hall Health and bring to Academic Services (HSB room T-310). It will be included in the info packet sent to the site.
2. Present a copy of your RN license to the hospital.
3. All NP students are required to obtain written patient permission to evaluate and treat. Further details will be discussed with your preceptor on-site.

The School of Nursing will provide to the site:

• Letter of good standing
• Copy of your CV
• Your student photo
• The immunization records that you bring to Academic Services (see above)
