1. Prepare an "offer letter" to GLMH, including the following information:
   • Legal name
   • Address
   • Phone
   • Date of birth
   • Social Security Number
   • Highest degree earned
   • The number of hours per week you will be attending this clinical site
   • The quarter's start and end dates
