All Students: The Basics - Student Identification Cards
UW Student ID Cards
Student identification (ID) cards can be issued any time after
you have accepted the UW offer of admission and have paid
the enrollment confirmation deposit to confirm your intention
to attend. Students who are eligible to register or who have
registered may obtain their cards.
To get your student ID card, go to the ground floor of Odegaard
Undergraduate Library (next to the By George Café). The library hours are 8:00 a.m. until 5:00 p.m., Monday through Friday. Please
bring state or federal issued photo identification (such as a driver's
license or passport) with you when you come to obtain your ID card.
Health Sciences Photo ID Cards
All students are required to obtain a Health Sciences
photo identification badge and wear it while within the Health
Sciences complex after hours, i.e., weekdays before 6am and
after 7pm and all day on weekends and holidays. Please fill out the
Photo Requirement Form
and go to T271 during drop-in hours.
If you do not come during the scheduled drop-in hours, you may make an appointment
by calling (206) 685-0332 (please note that it can take up to 3 weeks to get your
appointment), but you will be charged an extra $25 for this service. Students who
have their photos taken after the designated week of drop-in times will be issued a
building access/photo ID card the FOLLOWING QUARTER. Distance learning students,
call (206) 543-8736 if you cannot make it during drop-in hours.
Drop-in hours for Summer Quarter 2008 are as follows:

Please remember to bring the completed Photo ID Form, your checkbook and your UW student ID with
you to T271 (do NOT get your photo ID from HS Classroom Services). The information you provide
on this form will be used to identify you throughout your program of study. PRINT CLEARLY
IN INK.
You will receive you photo ID/access card from the School of Nursing during the 2nd-3rd week of
