Item 1: Human Subjects

Is the IRB review pending?

If you have not yet applied for human subjects approval, or if your application is pending, check yes.

If your project is part of a larger study and already covered by an existing approval, check no.

If you are getting data from another study/ database/ other source and will not need to apply for approval, but have not prepared a request for exemption, check yes.

Item 3

Proprietary/ Privileged information includes ideas for patents, trade secrets, or confidential commercial information for the applicant. If this information is in the narrative, than yes must be checked. Indicate this information in the narrative with a footnote stating: "The following contains proprietary/privileged information that (PI) requests not to be released to persons outside the Government, except for purposes of review and evaluation."

Item 4

Unless NEPA (National Environmental Policy Act) is specifically referenced in the application check no.

Item 5: International Collaborators

Indicate international subcontracts or support. The optional explanation should be no more than one or two sentences. If it must be longer, use the Other Attachments field below.

Item 6: Abstract

Use template.

It may not be any longer than 30 lines and must conform to font and margin guidelines of the rest of the narrative. It should include the aims, methods and research design.

Item 7: Project Narrative

Use template.

This should be one or two sentences about why this research is relevant to current health care issues. Use lay terminiology. The same format constraints apply .

 

Item 8

Upload the bibliography for the research plan. It should be in the same format as the rest of the narrative.

Item 9

Upload a document with resources in the school or university that the project will use. See form pages for an available template. The template has descriptions of all the resources available that anyone might ever use. Do not use the template in its entirety. Delete the resources that you are not going to use and add new ones (especially for subcontracts) that are not in there. Exclude Equipment.

You may use the resources template as a starting point for writing this section.

Item 10

Upload an explanation of major equipment that is already owned by the school that will be used by this project. Any equipment that will be bought with grant monies should be included in the budget justification. A pre-written list for all equipment available will be ready soon. Edit the list so you only submit the equipement that you will be using.
