PLEASE REMEMBER: you only need to collect once every three months:

On arising, the morning after starting the diary, before doing anything else, collect your urine specimen. No smoking, eating, drinking, or exercise before urine collection.

Fill in the test tube label: Date of Collection, Date started last period, and circle Y or N for any estrogen taken in past 24 hours.

Collect urine in the clean collection cup (no soap residue).

Pour urine into tube up to the line.

Secure cap.

Shake urine in tube to mix preservative.

Put in coldest part of your freezer in upright position in holder provided.

Urine sample must be delivered to RA within 8 weeks (56 days).

Do not put the collection cup in the dishwasher.
