Faculty Consulting for Non-UW Entities

If you are considering working as a consultant for an entitiy outside of the University of Washington please see the Outside Professional Work Policy. Once you have determined that this activity conforms to the policy, please fill out the Request for Approval of Outside Professional Work for Compensation form. Using the free Adobe Acrobat Reader this form can be typed and printed but not saved. To create a tempate which can be saved and reused, faculty must use the full version of Adobe Acrobat.
